WAIVER FORMS

We kindly request that all clients fill out the appropriate waiver form on the same day of their appointment to ensure the safety and efficiency of our services. It's important to note that each waiver form only needs to be filled out once per year per service. Whether it's for Brow Tint, Lash Extensions, training classes, or the Client general consent form (formerly known as the COVID liability form), we strive to provide a seamless experience for our clients. We appreciate your cooperation in this matter and thank you for choosing our services.

Off-Schedule service request form

Can't find the availability on the regular booking calendar? Fill out this form to request an off-schedule service. This only applies for appointments that are not on the booking calendar.

Before submitting this form, please make sure you have checked the availability for the regular booking schedule first. If the appointment is on the regular schedule this request will be denied.


Please keep in mind that there is no guarantee for availability for off-schedule appointments.


This is only an appointment request for a service that has no availability on the regular booking calendar. Upon approval of the request you will be given instructions on how to reserve your desired appointment and have to be fulfilled within an hour to ensure your time in secured. Please keep in mind, full payment to retain me for an appointment is 100%.

All sales are final. This appointment request is non-transferable and non-refundable


Due to the influx of requests please allow 1-3 business days for a response.
