We kindly request that all clients fill out the appropriate waiver form on the same day of their appointment to ensure the safety and efficiency of our services. It's important to note that each waiver form only needs to be filled out once per year per service. Whether it's for Brow Tint, Lash Extensions, training classes, or the Client general consent form (formerly known as the COVID liability form), we strive to provide a seamless experience for our clients.
