After booking this class, please forward a copy of your drivers license or state ID to bballc@icloud.com 2.As part of your certification, a barbicide certification is now required. If you already have the certification please send copy to the email address listed above. 3. Cancellation Policy

****For regular appointments: Cancellations/Rescheduling must be done a minimum of 24 hours before your appointment. Failure to notify during this period will result in fund paid being kept and a rescheduling fee of $35 additional to get on schedule again (if available) . ***Rescheduling must be done within 7 days after the date you originally booked otherwise all funds paid will be non-refunded. *** Class cancellations/rescheduling Must be done a minimum of 10 days before the class date. For classes you must find a new class date within 60 days for rescheduling from the day of your notification. Additional details of class info will be sent via email. If you cannot find a new date/time all funds will be kept. Cancellations after the 10day period will result in a $200 cancellation fee
